Evaluates patients with physical disabilities, plans and implements appropriate therapeutic and functional programs, and establishes appropriate goals. Provides care to patients with acute conditions, short-term illnesses, long-term disabilities, or terminal illness following a physician referral.

Staff Physical Therapist
Full Time / Part Time / PRN

Evaluates patients in their home settings, plans and implements appropriate therapeutic programs, and establishes appropriate goals to rehabilitate patients following a physician referral. Is responsible for the clinical supervision of the Physical Therapist Assistant and HCA in the management of the patient’s plan of care.
