Provides comprehensive nursing care in the patient’s home primarily after regular office hours including pre-planned visits and on-call responsibility. Is responsible for the clinical supervision of HCAs, LPNs, and student nurses in the management of the patient’s plan of care. Performs office duties and support case management responsibilities. Must have graduated from an accredited school of nursing. Based on geographic assignments, must be licensed to practice nursing in Indiana, Illinois, and/or Kentucky. Six (6) months nursing experience required. Six (6) months community health experience preferred.
