Donations

VNA Plus relies on charitable gifts to provide care to Tri-state area individuals who are sick, disabled or dying who do not qualify for Medicare or Medicaid, or who are unable to afford commercial insurance or private pay services. Your donation helps VNA Plus fulfill its mission of providing compassionate and cost effective services, and your gift is deductible on your federal tax return.

Donations may be made:

• Online:

• By Phone:
  Call 812-425-3561 or toll free, 800-326-4862 and ask for the Development Department Monday through Friday from 8:00 a.m. to 4:30 p.m.
• By Mail:
  Download a printable version of the VNA Plus donation form. Include this form with your gift. The mailing address is listed on the form.
• Gifts of Stock:
  Download a printable version of the VNA Plus donation form. Please indicate the name and number of securities to be transferred as well as other identifying information and mail this form to the address listed on the form. This will help us acknowledge this gift appropriately for your tax purposes.
• Planned Gifts:
  VNA Plus would be happy to work with you or your financial planner to create a legacy gift that meets your current and future needs. Please call 812-425-3561 or toll free, 800-326-4862 and ask for the Development Department Monday through Friday from 8:00 a.m. to 4:30 p.m.
